When „Close Enough“ Floods a Facility

Would you delay your project to question a connection that already looks technically acceptable? 


A flange connection that passed installation failed shortly after startup.

On this project, a DN25 drinking water line was installed using EN and ASME flanges. The parts could be assembled, but the different geometries and sealing surfaces were never intended to operate together under real conditions.

What physically fits is not always technically compatible.

Under operating conditions, the mismatch led to:

  • the flange connection failed under operating conditions
  • around 260 m³ of water spread into areas containing electrical equipment
  • affected systems required technical verification
  • the project team moved from execution into incident response mode

Projects can continue with assumptions. Systems usually cannot — and that is when schedule delays, investigations, equipment verification, and recovery work began.

“Almost compatible” is still a risk.
