ConQuality was invited at the final stage of construction of a waste-processing plant in Germany to organize mechanical completion and prepare for commissioning. However, on the very first day our quality engineer uncovered systemic issues that ruled out any possibility of successful project completion. There was no safety culture on site: scaffolding had been erected improperly, without guardrails or approvals, and personnel were working without personal protective equipment. Contractors lacked welding qualifications (including EN ISO 9606-1), there was no certification of companies as welding manufacturers (EN ISO 3834), no classification of piping in accordance with the Pressure Equipment Directive (PED), no supervision of non-destructive testing, nor any preparation for pressure testing in accordance with AD 2000. The client’s supervisory staff failed to recognize even basic non-conformities.
Despite the serious violations uncovered, the ConQuality team has put forward a practical plan to restore the fundamental conditions of quality, safety, and regulatory compliance.
